HOW TO ENTER: There are 2 ways to enter this sweepstakes: A.) Be willing to complete an "Internet Survey" in its entirety, which will be provided by the program Sponsor. Once you complete the survey and submit, you will be "automatically entered" into the sweepstakes. B.) To enter without completing the online survey, please hand print your name, complete address, phone number, and e-mail address (optional) on a 3 x 5 card and mail it to: "Aveeno Online Survey" Sweepstakes, 625 Panorama Trail, Bldg. #2, Suite 100, Rochester, NY 14625. Limit: One entry per person, regardless of method of entry. Mailed entries must be postmarked by 7/1/09 and received no later then 7/8/09. All entries become the property of the Sponsor.
ELIGIBILITY REQUIREMENTS: Sweepstakes open to all legal US residents, age 18 or over. Employees of Johnson & Johnson (the "Sponsor"), their affiliated companies, parents, subsidiaries and advertising and promotion agencies, (collectively, "Sweepstakes Entities"), and their immediate families and members of their households (whether or not related), are ineligible to participate in this Sweepstakes.
WINNER SELECTION: On or about 7/15/09, independent judges will select a single prizewinner in a random drawing, from among all eligible entries received. Winner will be contacted either via e-mail or via regular USPS. Entries considered will be based on the date the entry is received online, or the date upon which mailed entries were received. Judges' decisions are final. Odds of winning depend upon the total number of eligible entries received.
PRIZE: One (1) Grand Prize of $500 will be awarded.
GENERAL CONDITIONS: All taxes on prize, if applicable, are the winner's responsibility to pay. Winner may be required to sign an Affidavit of Eligibility/Liability and (where permitted) a Publicity Release and return it within 10 days of notification or prize may be forfeited and an alternate winner selected. By entering the Sweepstakes, entrants agree to all terms and conditions of these Official Rules, and by the decisions of The US Sweepstakes & Fulfillment Co., which are binding and final. Acceptance of prize constitutes permission for Sponsor to publish winner's name and/or likeness, if it so chooses, without additional compensation, unless and except where prohibited by law. By entering, entrants agree to hold harmless Sponsor and all parties associated with this survey and Sweepstakes from any and all liability arising from, or in connection with acceptance of the prize offered. Any attempt by an entrant or any other individual to deliberately damage or undermine the legitimate operation of the Promotion is a violation of criminal and civil laws and should such an attempt be made, Sponsor reserves the right to prosecute and seek damages from any such individual to the fullest extent permitted by law. Except where prohibited by law, all issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Promotion, shall be governed by the laws of the state of California, without giving effect to any choice of law or conflict of law rules that would cause the application of the laws of any other jurisdiction. This promotion is void where prohibited.
LIMITATIONS OF LIABILITY: The Sponsor and its agencies are not responsible for entries that are not received due to technical/transmission difficulties beyond their reasonable control, nor for late, lost, incomplete, misdirected, stolen, mutilated, illegible, or postage due mail-in entries. If, for any reason beyond the reasonable control of the Sponsor, the online portion of this Promotion cannot be implemented as planned, Sponsor reserves the right to terminate, modify or suspend the Promotion, and select a winner from all eligible forms of entry received up to the time of termination. Use of computer programs and/or other automated devices to enter Promotion is prohibited. Normal Internet access and usage charges imposed by your online service will apply. All personal information will be treated as confidential, and not sold or given to third parties.
